Did anyone else think when Dominus said:

"This world is an illusion, Exile." he was referring to the fact the Exile is doomed to repeat themselves in the next difficulty (e.g. Roland from The Dark Tower) and not the fact that he was about to turn into a giant demon?

I realize difficulty is a game play mechanic but story-wise it has some serious philosophical implications for the world of Wraeclast and who or what the Exile really is (Death, maybe?)

I thought it was opening up the possibility that future acts could take place in some entirely different "real" world.

I sincerely feel the Exile represents Death (e.g. Change/the end of an Old Empire or way of life). Their primary means of interacting with the world is destruction. The only help they give others is incidental to the killing they've done.

Certainly they might do something (medicine cabinet for example) that helps others but what always achieves this is death. There is, simply nothing the Exile does that is constructive, they represent pure destruction.

And more interestingly, to what end? They exile moves forward, destroys more, endlessly, for no discernible reason, they're simply driven to destroy and conquer.
Dominus fled across the Twilight Strand, and the Exile followed.

"reality is an Illusion exile"

This has many possible meanings in the world/realm of wraeclast.
However this one is my favorite interpretation:

All of the exiles are not actually in the real Wraeclast but rather lost in the "map" realms of malachai's machine. Those end game maps you are doing are actually just variations of the realms. Wraeclast is actually an empty hellhole with the sole exception of the Solaris Temple. Dominus Knows this and part of his "power" base is that he has learned to manipulate the map realms that the Exiles are trapped within. The problem is that the exiles have a chance of releasing various trapped beings back into the world.
The truth of Piety's "redemption" wasn't just a new name but also given dominion over the map realms of wraeclast to do as she saw fit.

This raises the question of how they access the Map Machine, although it could be said Lady Dibella owns a portal to it with in her temple, explaining why Dominus and Piety are at war with her.

Perhaps the world outside the map machine's room is real and we have no way to escape it.

Not really, it implies the world in general or at least the one the Exile and Dominus are in is an illusion/fake. It's a sensible statement (though the validity of the statement isn't confirmed per se)
