I'm an Infernalist. There aren't any passive skills, items, or gems that consume health. I was killed by monsters. However, my energy shield was almost full while my health dropped to zero.

I used to think this was a bug until i looked up what mob I was fighting against.
River Hag.
They have an ability that creates a water blob. If you stand in this water blob for 3 seconds your character will drown, draining your life to 0 while keeping your energy shield full.
The counter-play is to stay aware of your surroundings and never stand still for more than 1-2 seconds. If you see River Hags or watery blobs following you - kill them ASAP and don't stand still.
I'm well aware of this situation, but that's not what happened in my case. I was killed by this monster during the story - mission phase, but the subsequent incidents were not due to this reason.
Are you sure? After you first encounter them in the campaign you can also encounter them randomly in maps.
I'm sure I wasn't killed by the water ball. Because when you're inside the water ball, a blue border effect similar to drowning will appear around the screen, and the character's speed will be greatly reduced. If you don't get out after two seconds, you will die. Moreover, I didn't encounter the small monster that releases the water ball, and there was no water ball either. I always stay away from such monsters, but it wasn't the case those two times. I'm quite certain.

This is rather far-fetched hypothesis, but look at "prevents recovery above 50%" monster mod. From poe1 we know that:
- CI counts you being "at full life" (to prevent abuse of permanent low life bonuses)
- Poe engine uses int calculations for combat, meaning character health may actually be an int.
If true, an oversight may exist where 50% of 1 life rounds down to 0. In any case, pay attention to exact monster types that killed you, it may be they have skills that partially penetrate ES with physical dmg, and you don't have MoM. Judging from patchnotes released so far, poe2 is buggy as hell, so playing hardcore in this state is very brave:)

However, the negative effect of MoM, which halves the mana regeneration, along with the other factors mentioned above, is like a debuff that makes the gameplay rather uncomfortable. If I don't get a proper explanation in the end, choosing MoM will be a last resort as I have no other options.
Secondly, I'm considering not taking the Chaos Inoculation talent and instead focusing on stacking Chaos Resistance and Life. I might even incorporate MoM in this setup. This could potentially be the most effective way I can think of to counter this type of damage that directly hits Life through the Energy Shield, which I can't quite comprehend. Nevertheless, this will undoubtedly consume a vast amount of my affix resources to stack Life and Chaos Resistance. And even if I manage to do this, my effective health against damage that penetrates the Energy Shield won't increase significantly. This is because both Life and Mana are severely restricted in Path of Exile 2. Since I'm playing as an Infernalist, it's hard to feel at ease. When you see only a little over 1000 health points and meager mana to resist damage that penetrates the Energy Shield, it's bound to make you nervous.

I may be a newb at numbers of hours under 500 for both games, but I think CI doesn't make sense to get unless you already have a massive ES / mana pool.
I tried it and didn't like it, but I only have about 3k ES and 3k mana. 2k life.
From my POV it's easy enough to avoid most chaos damage and getting 10-20% chaos res is enough for at least softcore gaming where you want to be powerful enough to destroy bosses before they go into bad phases.
Basically I'd rather have 2000 life and vulnerability to chaos than 1 life and very twitchy mana pool, ES pool that can be one shot gone, two shots dead kind of thing.
