I believe that is act4, the layout have been found in the game files a long time ago, so unless they are keeping old layout of act 3 town in the game files for no reason, that is act 4 town.
+ they said they will send concept arts "from the past, present and future of path of Exile" what else "from the future" do you see in the concept arts? Nothing :)

Well as far as i know Act 4 is rumored to take place in the highgate mountains (where all the gems come from). Only question for me is: How do we get there. All the acts are physically linked. You never "warp" from act to act. I really hope GGG doesnt make it so that you enter the portal on the sceptre of god and zoosh! your in highgate. Keep it consistent, otherwise it would kinda riun the immersion (like Dark Souls 2 did).

Looks similar to the wireframe that was found a long time ago, so I think it's legit.

When hsing was leaking info about act 4 we got a glimpse of areas to expect.

Looking at this map I imagine we're going north of the docks/sceptre across that long aqueduct, which has been in concept art of the game since forever. That's probably what the "grantori vitalus" is. The dried lake is from where the aqueduct fell and flooded the north of sarn, probably destroying most of it. Then we will reach highgate from there.
