Are you sure PoE is using your dedicated graphics device? Some laptops have been known to use the integrated graphics instead.
It's usually possible to force the game to run on the dedicated device either in your graphics device control panel or by using one of the other tricks people have documented in the forums (and we're also working on a solution to make this unnecessary in future).
Im not sure about the dedicated device stuff, but when i run the game in option the graphic card is shown as Radeon HD 5730 which is the card my laptop is using.
How do I check if the game is using my dedicated graphic card?

No it isnt, your laptop has a 6570M inside of it, that 5730 is a shitty onboard adapter. The 6570M will mutilate the 5730 in every way.

It actually shows "AMD Radeon HD 6570M/5700".
So what do I have to do now, I'm not sure how to make a profile or how do i set my laptop to run games with 6570M graphic card?

I think you forgot or didn't know that the the ATI/AMD 6570M is the same card as the 5730 with a re-branded name and better memory support.
As it stand the OP seems to have the same issue I am having with seemingly unexplained FPS issues. Since I am also using AMD video cards it seems like the game isn't optimized with our cards. And just like me, changing the setting makes no difference. Max setting, or Min setting I get the same FPS drops which baffle me.
To OP: Besides updating your drivers, keeping an eye on your gpu temps, and playing carfully I don't know what else to tell you. Hopefully the game goes platinum, and AMD makes some dedicated profiles/updates the drivers or GGG adds some better support for us. I bet GGG comps are using Nvidia cards, no love for AMD.

GPU temperature wise I have no problem since i can cap my FPS @ >30. I tried capping my FPS range within 30-60 but it makes no difference on spikes.
I admit though, all games are base around Nvidia cards, but I can't help it, bought this laptop 2.X years ago on impulse, am not gonna buy a new laptop with AMD support cards.
