Hammer of Kamasa does not drop from Act 3 The Molten Vault Boss

I have a main that has completed the campaign without issues. I'm running a second character with my wife and we've gotten to the "Treasure of Utzaal" quest in Molten Vault.
A hammer quest item drops which did in fact drop for my wife (i believe I was the party leader at this time) but it didn't drop for me. I swear i heard the little sound it makes when a quest item drops but it wasn't on my screen.

I ran to town with her, she handed the quest in but nothing happened to my quest, I still of course didn't have the hammer.
I ran the Molten Vault again in a new instance, alone without her in my party and once again the item didn't drop. I should say however, this time I am sure the quest item sound went off. It almost felt like it spawned under the ground.

I have since ran it a third time in a new instance and yet again, it didn't drop. I assume I can't progress this character until this is addressed.

(Leading up to the original kill of this guy with my wife, when we "zoned in" to the area by clicking "Arena", we spawned on top of each other and couldn't move until we both dodge rolled, I don't know if this is related or not to the issue.)

If you already got the hammer with a previous character in the same league, then you can ignore the quest - it is not needed to progress.
