Thirst for Blood is from Jack, the Axe and should be used. It provides a lot of bleeding damage for 10% life.

Ah okay thank you! I thought it reserved 10% mana, not life. So I was trying to figure out what aura would need to go.

What would the difference be fitting in Impale support instead? or maybe a better question is what is the reason for not using impale, is it less damage?.

Impale damage scales off of Hit damage and Impale Effect. Majority damage of this build comes from bleeding (~20% Hit and ~80% Bleeding) so it makes more sense to scale physical/ailment rather than impale. I do see people using Impale on PoE Ninja and I think Cruelty is a much better gem now that we can see the buff effect percentage.

I just added cruelty support on shield charge, as it can gives up to 40% more DoT and it helps during boss phases.

I got a few questions maybe you can answer:

1. For maximum dps, would you consider a double large cluster setup or is that too much?

2. When going for the cluster setup, what would be the skill points in the tree you would give up for?

3. Using PoB, is the most efficient setup for a 6L to be lacerate with multistrike, brutality, melee phys, ruthless and fortify? In pure dps number this looks like the best on PoB but maybe swift affliction or awakened deadly ailments feels better in-game?

4. How does the endgame look like for lacerate bleed gladiator?

If you hover over the Cruelty buff, it says skills supported by Cruelty deals x% more damage over time. So I think if Cruelty is created by Shield Charge then only it can benefit from it.

Isn't it better for the build to choose Entropy or Growth and Decay instead of Command of Steel? Command of Steel does not increase Bleed DPS but Entropy or Growth and Decay boost Bleed DPS huge. Tested in PoB.

Command of Steel was changed in 3.15. It previously increases bleeding damage. I did not update the guide yet. I think Growth and Decay is very strong.
