Cataclysm was caused by the Rapture device, map device is called Reverie. Dialla is mentioned because she was the "key" to destroying thaumaturgy, but she refused to sacrifice herself.

With this the case, then I'm pretty sure that Dominus second form really isn't Dominus but just one tentacle of The Beast. So it looks like it might be our fault its woken up. OP thanks for the interesting read. From the looks of it you might be actually correct.
For added info, in comic book issue no 4 there is a certain sequence where a certain historical character shows a manifestation of the "dark heart". A extra dimensional being that is supposed to be the source of all thaumaturgy. The image of its was very similar to the offical art picture you linked. Lots of gory tentacles etc. Now the character who showed that also displayed its malevolent intents after the cataclysm began. Finally now that the http://www.pathofexile.com/theawakening/actfour page has gone up, the section of "the beast" is currently locked but you can see a siluette of vaguely human entity with tentacles. My guess is that the historical character who depicted the dark heart managed to bind the extra dimensional being to him/her and thus has become kind of a avatar for the entity in the Wraelcastian realm. With the human turned into a anchor this event unleashed the cataclysm as the thaumaturgy realms influences started creeping into wraelcast. A'ka the wormhole theory OP referred to. Thus this avatar might actually be the physical manifestation we will be confronting in the final showdown of act 4?
