Why is there no custom UI options or API for modding the Interface?

Being able to customize the interface and add extra functionality like click through to view specific items, the ability to move where your buffs appear, the size of them etc etc.

Hell charge access to it. Like you can't utilize any of that stuff unless you spend like 5$ in MTX currency.

It would make the game significantly more enjoyable for many people. It would bring people to the game as well as I don't think games like Diablo 3 offer those sorts of options.

I can't imagine it would be all that difficult to implement? Think about how many years ago World of Warcraft implemented modding. I don't think I could play World of Warcraft with the vanilla UI at this point. I find myself getting frustrated with the user interface on a pretty regular basis just because I know it would be possible to make it a lot nicer and clean looking.

Also it seems entirely in line with the custom loot filters, like it seems something the developers would support?

I'll guess: When they made the UI they were a smaller indie company that didn't have the extra resources to devote to the idea. Matter of priorities. Now, it's probably too late to do anything major with it.

If you really want something like this, hope it's in 4.0. We really don't know much about their plans for it, but it has to be something pretty major to warrant the new series. Most think we're getting new character models. Probably that. Those were once "impossible" too.
It's never too late, it is, as you said, a matter of priorities. It won't bring them money so why bother. Better churn out more mtxes and new OP gems to shake the meta.

I'd love to get healthbars over or under my character... My screen is so god damn big I cant keep track of both globes while looking at my char. This setup worked back when diablo2 ran in 800x600 on a 15-17inch screen...

Probably not difficult per se, but it would take a huge amount of time.

I don't think they can.

If there's any chance the ability to make changes to the UI could help people with various eyesight issues, and GGG monetized that, they'd be seen as taking advantage. They would be crucified.
