Exhausted Trying to Fight Lag Spikes

This is my last resort, I've run out of options. I've tried everything I can possibly think of to fix this issue and I believe I am just going to be stuck in an ask them loop. My ISP says it is the game and I am guessing that any help here will say that it is the ISP, but here goes nothing.

I am experiencing lag spikes. After a short time Googleing, I had found out that one of the possible reasons for my lag spikes was a program on my computer called Asus Gamefirst III. Upon uninstalling this application, I noticed drastic improvements all around. My BIOS bootup time dropped down to 5-6 seconds from upwards of 30, NVIDIA GeForce Experience load time jumped from literally 2 minutes to around 15 seconds and most importantly, lag spikes in Path of Exile dropped down significantly, but still persisted from time to time. After a few calls to my ISP and some failed attempts at trying to fix the issue via port forwarding, resets and cable swaps, a tech was called out to my house to address the issue. He has spent the last couple of days here trying to fix the issue and we have even gone as far as swapping out the modem. My internet connection is ~150MBps. The issue pursits on the 2.4GHz WiFi frequency, 5GHz, and hardlined into the PC. It is present on all computers that I have that can run Path of Exile, but he is convinced that it is not on his end despite me tethering the PC to my phones LTE connection (I believe this to be the nail in the coffin, it is either the hardware in the house, or the ISP's issue) and not experiencing any latency issues or connection drops. He is unfamiliar with the WinMTR software and has me run a cmd window with the input "ping google.com -t" while playing. Ping is around ~30 - ~70 with the very occasional spike around ~100. None of these spikes in the cmd window correspond with the spikes that I see in the game, dual monitors with both windows up to check. While playing with my significant other, we both experience lag spikes at different inter volts and our connections drop at different times as well. I miss fully enjoying hardcore Path of Exile and would love to hear any advice from an outside source. Thank you for your time and I hope you all have a very merry Christmas.
It does sound like you have worked hard to figure out what is going on with latency spikes on your connection. If you can get a solid game experience tethered to your phone, but not through your home internet connection, that does sound like it is your service acting up on you. Have you tried running a VPN while trying to play PoE on your home connection? It may be worth a shot if the problem has to do with how your ISP is managing your connection.

Thank you for your suggestion, Max. After an embarrassingly long time and multiple failed attempts, I was unable to get a VPN connection working where I could successfully log into Path of Exile. I tried multiple programs and a few different servers through Windows 10's built in PPTP VPN settings. I am not too familiar with VPN's I should add. This was my first experience with a VPN.
I've actually been playing on my cell phone's LTE for an hour or so tonight and I've noticed that the lag spikes are continuing there also. Bluetooth tethering, USB tethering and straight WiFi tethering all produce the same results :(

There are no connection drops and time outs on the LTE connection, though. I can produce the issue on my significant other's laptop as well. Hers is an HP, mine an Asus. We have gamed together in the past without this issue. I am inclined to believe that it could possibly be an issue with Path of Exile, as much as it saddens me to admit. I've thoroughly tested many different configurations now, all producing the same result.

Try vypervpn in free version, select "fastest server" on client interface et let us know the result. This program is simple et work great with PoE ;-)
Thank you for your suggestion. I've tried that VPN, along with many others and lag spikes are still found no matter what I try. It is worth noting however, that my ping seems to be less sporadic and the spikes less frequent and drastic.
