level 91 newb to endgame dies with full energy shield?

Something is either wrong with my build (most probable) or there's a serious bug. There's no warning, just dead from 2k to 0 with full energy shield. Can anyone tell me what is causing this so I can continue to enjoy this otherwise great game?

Oh, I just noticed watching my video replay that it says I committed suicide. My mana flask does not take from my life, so that shouldn't be the problem.

I'm playing a hybrid ice wolf/wyvern and using a headhunter belt (I looted it in interlude so I've had it a long time and this only happens on corrupted maps). I'm guessing it's some interaction I'm unaware of but I can't figure this out and it has bummed me out, big time.

I hope it's just something wrong with my build that I can fix.

If anyone actually read this I think I figured out what is happening: an interaction with Paquate's Pact that I was using in my Arctic Howl! This is the only thing it could be as nothing else I'm using takes from my life.

It must be something to do with corrupted map mobs because I can use it on any other map and that doesn't happen.
