Sealed Vault Drop Bug

Just finished Sealed Vault and Bosses did not drop any loot.

P.S. Potential reasons for this issue could be:

1) Location were available even before I cleared all Checkpoint map, but I could not start it.
2) I died in one of the Checkpoint maps and did not activated the sigil as I had only 1 portal (it did not appeared again on second run).
3) I've put 3 Precursor Tablets to juice up rarity.
