With the 'blink' or random teleport effect/mod on HeadHunter, if it is active while you get in the Transcension Device it will teleport you out of the device to enemies to die, and you can't move or anything it bugs out. I don't appreciate how in PoE2, league 0.4, many players - including big streamers, abused the unintended holten mechanic and destroyed the game for so many others - all economy integrity was ruined and they live streamed it.
