Completed maps turn "attempted" and can't claim ascendancy points

hi i've been grinding today and noticed some of the maps that i've done this morning randomly were flagged as "attempted" even tho im 100% sure i have completed them(some were the only way of advacing), and before logging out for today i did my 4th ascendancy and at the very end, after speaking with the trialmaster and clicking on the ascendancy fountain i was unable to get my ascendancy points tried a fast relog but it sent me to the temple of chaos waypoint

My bad, misread the text and thought i had to only finish the run to be able to claim the 4th ascendancy, i now realize i have to defeat the trialmaster.
