"Tattoo of Hinekora" in Act 4 - Bugged Not Given

I didn't get "Tattoo of Hinekora" on one of my characters, and played it previously with 3 other characters.

On my last character the quest didn't give me the tatto as a reward at the end, even though I did everything exactly the same like on my other chars.
