Why items always puff when using the Orb of Chance or when recombining?

Why items always puff when using the Orb of Chance or when recombining? That's why we call the Orb of Chance as Orb of Puff. I am aware of the existence of omens, but the result is such a thrash that it is unclear why these mechanics exist in the game. Probably to get rid of items? :)
