Poe2 Xesht not spawning after completing Breach in Twisted domain.

Did a 100 splinter Breach run in the Realmgate and after completing the breach, got spawned back to the start and got all the loot and thats it. Xesht did not spawn in and i couldnt go through to a next room or so.
