I died and want to start another one, but the previous instance is still opened and cannot continue ....

Not sure what happened, but the instance should go away after long enough inactivity. I think it is 20-30 min.

Go to a waypoint and then go to the temple of chaos, but use right click (or hold controller button (A)) to start another instance.
