Omen of Amelioration not Working

I had only my omen of amelioration equipped but not working. Loses 10XP at Level 93! This is frustrating!

It has happened 4 times to me. Sometimes it works and sometimes it does not. I hope GGG fixes this!!!

I had this issue a lot and then realized that Omen's only work once per instance. Perhaps the issue is that you are dying on the same level twice? I believe the work around is to leave and start a new instance/level if you've already died once/used an omen on a level.
