Same thing. I have just this omen active, and when dying in rituals to chaos damage sometimes it bugs and does not work. Fix PLZ. Sucks losing so much XP at lvl 96.

Omen didn't trigger 3x in a row. Even traded it to stash, grabbed different ones, selected and unselected. Just isn't working.

Bump as this is still an issue.
The omen doesn't work even if it's the only omen active.

One shotting players leaving them without any clue what happened is still a thing in 2025, cause GGG can't move any step away from 2013. But selling useless items that have the purpose to prevent multi cluster effs regarding one shots is pure player hate.
