Omen of Amelioration did not work

Copper citadel at boss. The mechanic where you need to stay in the shield ( Have done it many times already ) All of a sudden ONE SHOT death ( No idea what even killed me ) > 10% EXP loss, omen still in inventory

Anyone having similar expirience? T

1) You didn't right-click to activate the omen
2) You had another activated omen that got triggered instead

Valid points, I know what happened, there was Resurgance Omen activation in the instance, which is what you meantion in point 2.
