Act 3 Cruel Bog Witch still bugged

Hello,

I have ran the Bog Witch nearly 2 dozen times, and she has yet to drop the item to grant additional spirit. I have went back, ensured I have completed all quests possible (minus the other bugged ones) and nothing. I am aware that it works for others but I do not receive an item to grant additional spirit. Please advise.
