If you leave the instance before the animation end and item drop, it will become stuck upon revisit.

Reproduction steps:
Open cortex for example.
Clear the map and kill the boss.
Open portal before final animation ending and hop in.
Revisit the instance in few seconds - no portal out of the boss room, no items, only the portal to hideout remains.
