Always having an Unknown Disconnect!

Hey I need help, since Scourge I always have difficulties with Disconnects. Like, when I'm Going in a new zone/map after ten minutes or so im getting disconnected to the Log-screen. before that happens, my Ping is going higher and higher until the 5000's or more but i checked my Internet and it was fine and since it only started with scourge i think that the game is fault for that.
I NEED HELP!
