Continuous disconnects when changing instance

Hello,

I'm having this recurrent problem since the league began and it's starting to drive me very mad. It happens to me at least 2 or 3 times every day, and it appears when I take a portal to a different instance, and after a long loading screen, I'm thrown to the login with the following message:



How can I solve this issue?
